Choice Health at Home Acquires Alliant Home Health, Palliative Care, Hospice
Choice Health at Home has acquired Alliant Home Health, as well as Alliant Palliative Care and Hospice. Financial terms of the deal were undisclosed.
The two acquired businesses were part of Colorado-based Alliant Living, which provides hospice, home health and palliative care. Alliant Home Health’s services include occupational and physical therapy, as well as skilled nursing care for patients post-acute and chronic conditions.
The acquisition expands Choice Health at Home’s existing geographic presence in the state. The transaction was recently completed following its closure on November 12, 2025.
“At Choice, our focus is on building integrated, high-quality care platforms that truly serve patients and families,” Choice CEO and Founder David Jackson said in a statement.
Texas-headquartered Choice Health at Home serves patients in Arizona, Colorado, Kansas, Nevada, Oklahoma, Utah and its home state. Founded in 2007, the company is backed by the private equity firms Coltala Holdings and Trive Capital.
The company’s home-based programs include hospice, home health and personal care, among other services. Choice Health at Home also has programs for congestive heart failure, chronic obstructive pulmonary disease (COPD), fall prevention, stroke recovery and multiple sclerosis (MS) and amyotrophic lateral sclerosis (ALS).
The Alliant acquisition marks the first deal announced by Choice Health at Home thus far this year. The company in 2025 picked up Houston-based Devotion Hospice and Family Tree Private Care, which serves Texas, Colorado and several southwestern U.S. communities.
Helix Health Capital Advisors served as the financial advisor to Alliant for the transaction. Founded more than a decade ago, the company became Medicare certified in 2022 and serves a predominantly urban patient population.
The acquisition will allow for strengthened sustainability and scale, according to Tim Heronimus, co-founder and principal of Alliant Home Health and Alliant Palliative Care and Hospice.
“Joining Choice Health at Home allows us to build on the strong foundation we have created for patients, families and our employees,” Heronimus said in the statement. “Choice provides additional resources and support while preserving the personalized care and local leadership that define who we are. We are excited for what the future holds for our team and the communities we serve.”
Similar to national trends, Colorado is seeing rising demand for hospice as aging populations swell in the state. Seniors 65 and older currently represent 16.4% of the state’s overall population, reported the U.S. Census Bureau. The number of individuals in this age group will rise by more than 46% statewide by 2035, according to a report from the Colorado Fiscal Institute.
Hospice utilization rates reached 49.03% among Medicare decedents in Colorado in 2022, which fell in line with national averages that year, the National Alliance for Care at Home reported. Utah saw the highest hospice utilization at a rate of 59.61%, according to the Alliance report.
